Hi, if I attack with Kytheon, Hero of Akros and two other creatures... is it required that all creatures survive for him to transform into a planeswalker? Or he still transform even if the other two get killed?

Not only will he still trigger if he is the only survivor of combat for you, it's flavorful for them to die
