If I have both of these out will the single Kotis could as both a blue and black permanent and get +5/+5?

Yes. Ana Sanctuary doesn't care about having different permanents for each color. It just cares that you have a Blue Permanent (Kotis) and a Black Permanent (Kotis again). You meet the criteria so the target will get +5/+5
