Yes. The way the casting process works, the reduced cost is locked in before you have to activate mana abilities to pay for it, By then, it doesn't matter that the Crawler doesn't have 4 power anymore.

Absolutely fine and dandy and commendable, as long as you don't write on these pieces of paper prior to a game in progress, only while a game is in progress, and you don't put these pieces of paper inside of sleeves and shuffle them in.

Your assumption is correct, it's all one triggered ability, so nothing else will go on the stack until the ability is done resolving. So a madness creature discarded to the 1st mode will not be on the battlefield to be sacrificed to the 3rd mode.
