The black saga depicts Elspeth's suffering at the hands of Phyrexians during her youth. Ashiok uses her frequent nightmares of Phyrexia against her. This is what this saga depicts.

Maybe Elspeth's nightmares are made visible in the world to others through Ashiok manifesting them as Nightmare creatures. Maybe the artist got insight through some divine visions.
