Let's pretend you somehow have only insects remaining in your deck. How? Who cares. It could just be that the last 4 cards in your deck are all 4 of your Scute Swarm copies. You have Wheel of Sun and Moon cast on you and you +1 with Grist, the Hunger Tide.

What happens? Infinite mill or mill 4?

Assuming this is infinite mill because Wheel is a replacement effect, this then causes a draw because Grist's re-mill effect is not optional.

722.4. If a loop contains only mandatory actions, the game is a draw. (See rules 104.4b and 104.4f.)

Why bother with mere rulings when so many answers can be found in the Rules?
