Equipped creature has base power and toughness 6/5 and gains annihilator 1 and cumulative upkeep .
As long as the equipped creature has an age counter on it it gets +1/+2 and gains "When this creature attacks, lose 1 life"
As long as the equipped creature has three age counters on it it gets +2/+2 and gains "When this creature attacks, lose 2 life."
As long as the equipped creature has five age counters on it it gets +3/+2 and gains annihilator 2.

Relics of the Eldrazi are not a pleasant sight, even if you are wearing them or not.

(When you've paid 15 mana for the creature its base power and toughness are 12/11, it has annilator 3, and "when this creature attacks lose 3 life".)
