As long as the flavor of the individual cards is just right, I think switchform could be a really great transform mechanic.

Oh, I really like this too. Mixing +1/+1 and -1/-1 counters, of course, is a bit of overcomplication that's justly avoided in real cards, but I've been guilty of it in my own custom designs in the past.

Thank you, switchform was born out of two needs; how to get plenty of transformation without needing to track day/night, and how to make transformations become drawbacks. There's a lot of room for it, like Hibernating Bear/Grumpy Bear, Infected Farmer/Unstable Werewolf and the like.

Woodwasp was because I really wanted a wood-boring insect that eats Forests to grow bigger, but making it worthwhile to sacrifice lands without actually making it broken was difficult. Nothing more frustrating that felling a couple of Forests only for your bug to get squashed.

Persist solves that nicely, while adapt keeps it restricted.

The Haunt counter is throwing me off because it's not doing anything. It turns GY creatures into aura enchantments, which is weird to represent. How about, "Exile target creature card from your graveyard under target creature. When this creature dies, put the exiled card onto the battlefield." Wording needs cleaning up but the counter is unnecessary

The haunt counter is mostly just a reminder that this exiled card is haunting a creature and has a triggered ability when the creature it haunts dies. Like the manifestation counter from Arbiter of the Ideal or the ghostform counter from Kaya the Inexorable.

But from a design/Mel point of view, it's also a safety feature; in case the rules require that a card must have the haunt keyword to haunt something, the haunt counter can also acts as a keyword counter granting the card the haunt keyword. This is just me hedging my bets and covering as many bases as I can.

In short, if the rules need it, the haunt counter acts as a keyword counter. If not, then it's just a reminder counter.

The haunt mechanic already exists, so this in fact is easier to use than the mechanic you suggest.
