Damage on creatures is not removed at the cleanup step.
Tormented Flesh gets +X/+0, where X is the amount of excess damage on it.
Whenever Tormented Flesh or another creature dies, that creature's controller loses life equal to the amount of excess damage on it.

@bravelion83 My intention with the prompt was more to the point of what netn10's entry is (like a fundamental change to MtG's core rules), but I don't think that should matter haha. I think the beauty of New Challenge Every Day is that everyone gets to take their own spin on not just the prompt, but also the interpretation of the prompt i.e. death of the author.
