Form of Phyrexia
Enchantment
As Form of Phyrexia enters the battlefield, your poison counters become ten.
If you would gain or lose life, you lose that many poison counters instead.
At the beginning of each end step, you get twice as many poison counters as you lost this turn.
You don't lose the game for having ten or more poison counters.
When you have no poison counters, you lose the game.
When you have forty or more poison counters, you win the game.

Form of the Elements

At the beginning of your upkeep, choose a form:
* Form of Nature - Your life total becomes 10, then you may put four land cards from your hand onto the battlefield.
* Form of Arcane - Your life total becomes 5, then draw four cards.
* Form of Flames - Your life total becomes 1, then you deal damage to any target equal to the number of cards in your hand.

Enchantment (Mythic)
When Form of the Eternal enters the battlefield or at the beginning of your upkeep, Amass 5, then your life total becomes equal to the power of the Army you amassed. (To amass 5, put five +1/+1 counters on an Army you control. If you don't control one, create a 0/0 black Zombie Army creature token first.)
If a source would deal damage to an Army you control while it has any +1/+1 counters on it, prevent that damage and remove that many +1/+1 counters from it. When you do, that source's controller discards a card.

Form of the Ooze enters the battlefield with X slime counters on it.
At the beginning of your upkeep, your life total becomes equal to the number of slime counters on Form of the Ooze and Form of the Ooze deals that much damage to any target.
